I would like to have a quiz on our homepage that is similar to what is on nutrisystem.com’s website — their ‘free body assessment’ on the bottom right. I would like the quiz to calculate the customers BMI and, based upon their answers, suggest a plan appropriate for them… (Budget: $30-$250 USD, Jobs: CSS, Javascript, MySQL, PHP, Script Install)
