Rma Application For Cubecart 4 Website

We have a website that needs an RMA application built and integrated into Cubecart 4.

The requirements of the app are as follows:

We need to determine if the customer is logged into cubecart or not.
If the are then (2)
(1) Customer must register with the site and log in – go to (2)
(2) Have a page the customer can go to and fill out a form with the following fields:
Name
Address
Email
Phone
Product to be returned:
Problem with the product
Date Purchased
Where purchased

Once submitted, the app will log this data and generate a unique RMA Number. Then a page will be displayed (in a popup window) that will contain a shipping label with the RMA on it as well as instructions for the customer to follow (I can provide you with a graphic for this).
The customer would then print it, affix it to a box and ship the item back.

In the admin section, we need a section for RMA’s
In this section each RMA will be listed on a line. If you click on the RMA number you will see the rest of the info (outlined above) with the addition of the following fields:

Status
Tech comments
Action

The Status would be a dropdown with the following options:
Waiting for Item
Item Received
With Technician
Shipped Back to Customer

The Tech comments would be a text area field where they can make notes on what was wrong

The Action will also be a drop down which will have the following options:
Will Repair
Will replace
Will refund.

The last feature of the app will be a login section on the RMA page of the website: http://www.viper-rc.com/store/index.php?_a=viewDoc&docId=54 Where the customer (once logged in ) can check the status of any open RMA’s.

we do not have a large budget for this, but we do need it done quickly.
