Its a large form [please refer attached xls file]. Data to be stored in MySQL database, and also email to an email id.
Please refer to the attached file, there are 4 sheets, I already have created the mySQL table for sheet 2 [largest form among 4].
So your job will be
create Mysql Table to other 3 sheets,
Create form for all 4 sheets
Form validation
On submit form data to be stored in Mysql Table.
Create a backend system to see the data,sort & serchable by customer.
Escrow will be done, but no advance payment, you can do it in your server and show me the fully functional product.
