I have a form with a popup menu and radio button option for “yes” or “no”. If user select “yes”, i would like for a textfield or textarea to dynamically appear beneath it.
I have attached 3 files:
1)form1.png – what the form should like when “no” is selected.
2)form2.png – what the form should look when “yes” is selected.
3) sl1b.cgi – this is my perl cgi file with the form
** Please add the required JQUERY code to sl1b.cgi to make this possible. Should be fairly straightforward.
