I need someone to fix my custom .php script. It used to work but for some reason, its not working anymore.

And this script creates custom float up ads that display autoresponder signup forms.
My problem is that the autoresponder signup forms are NOT working.

I will need someone to fix this in the script so the autoresponder forms work. I have also attached a text file of the javascript code so you can test it for yourself and see that its not working when you try to submit the forms.
