I need a simple form mailer writen in php using mysql back end. Basically the form will be viewable online with the following field:
Full Name:
Tel:
E-mail:
Company Name:
Request Detail:(Text Area of 400px X 300px)
With a “SUBMIT” button. That’s it.
The backend part of the form is as follow:
The input of the form is to be routed to a list of email address already on a sql database. The admin part of the form should allow access to admin to upload a csv file containing the email addresses for updating purposes.(See attachments below)
Address Upload Field: (csv or text file with email address)
Should be capable of handling a large volume of emails.
