You will create this iPad application where the user will complete the attached checklist, about 30 questions with yes/no answers. After completing the form,t he user will hit submit (or cancel) button. The applications will generate (preferrably) PDF forms with all inputted data. If you can add a simple graphic (logo), then that would be nice.
