I have attached two pdf form files. I need these two pages to be recreated into an online form.
Each field needs to show a tooltip…http://bit.ly/bd1ZXF All fields need to have a [?] and if hovered over it will display tip. I will enter all info tips inside the div(s) (onMouseout=”hideddrivetip()”> I will write tips</div>) You just make sure all fields have [?] to the right on it and room for me to type out the tip.
Form needs to have:
-autosave
-remember cookie (in case user closes screen by accident)
-#1 on first page form A, B, C, D, E need to auto calculate numbers total when entered.
When you click submit the form will create a PDF file or the document with entered information.
Needs to be formatted exactly the same as you see. Should come out on two pages just like you see.
