Urgent: Php Form Made And Added To A Simple Php/mysql S 2

I have quite a lot of work to be done in PHP.
The first one being a short form, which will take around 15-20 values in a form and save them to a MYSQL database.

Have lot of forms to be made into that system. Its a very simple PHP/MYSQL system.

so less than an hour’s work.
$10 budget for this small form and immediate more work to follow.
