I need a reliable programmer/coder to create a simple PHP script that allows users to upload photos to my host via jquery/ajax interface.
The upload form should have fields for the user to insert a Title and Description for the image.
The title, description, and image filename/path should be stored in a mysql database.
The image should be stored on the server, suggested path is:
/imgs/mth/$userID-filename.
File format allowed are: jpg, gif, png ONLY.
