I need a page which will take First Name and Last Name and email id as compulsory fields and phone number address and city state zip as non-mandatory fields.
iPad will have no wiifi or 3G .
Needs to store info in iPad and can be retrieved later when connected to internet or in wiifi.
Need to put my logo and customize it as per my need .
