We have paper forms (in PDF) and HTML forms that we need to manually convert to Word templates with MailMerge place holders. We don’t need too much design. We basically want a recreation of the original form in Word… (Budget: $8-$15 USD, Jobs: Format & Layout, HTML, Word)
