We need a simple submit form app for all mobile devices. This app will keep a profile of family members, name/age/dob and a couple other simple demographic info. config also requires a special id reference, which will query a mysql data for other basic submitting information… (Budget: $30-$250 USD, Jobs: Android, Cocoa, iPhone, Mobile Phone)
