I need a contact form to be able to send information to my email using php.
1.It will just contain two fields: “name” & “email.” Once form is submitted, a pop up saying “Your information has successfully been submitted, God bless.”
2.The site is located at presenttruthmission.com, and you can view the form on the index.html and the register.html pages. They are the same form that I need in both of those pages. (right now it is not functioning)
