I am looking to convert a complex PDF form to a HTML Ideally i would like to keep the look and feel of the PDF file but make it HTML with javascript to clients and fill it in online. The results will need to be sent via email in a presentable way… (Budget: $30-$250 USD, Jobs: HTML, Javascript, PDF)
