I need a website with less than 10 pages (including sitemap, privacy page and contact us page). The main part of the site needs to be an online form for people to enter around 25 pieces of personal information (name, date of birth, occupation etc) that will be sent to one of my email addresses… (Budget: $50-$120 AUD, Jobs: Graphic Design, HTML, PHP, Website Design)
