Simple aplication in php, there will have 4 pages. First Page- Register Pacient Will have 9 input fields, one of the filds will have a dropdown list menu of the exams. the list menu of exams will communicate whit a table in the database called for exame “type of exams”… (Budget: $30-$250 USD, Jobs: HTML, Javascript, MySQL, PHP)
