A Multi-step form (HTML/CSS) is currently operational. There is a bug, however, when a particular selection is chosen where one of the checkboxes becomes hidden. Also, a slight variation to make another form field appear is need for this… (Budget: $50-$75 USD, Jobs: Javascript)
