Lightbox Syle Contact Form And Standard Contact Form

1.) When a user clicks a ‘Get a Quote’ button a lightbox style Contact form opens with the following form options;
– First Name
– Last name
– Location
– Email Address
– Enquiry Type – 1.General; 2.Residential; 3.Commercial; 4.Other – Enquiry (text box)

2.) Same conatct form but for the contact page.
