I have a new site I am building from html and php. I am not very good at mysql yet. I want to add an appointment page to my site. The form will ask for certain information with some questions being mandatory… (Budget: $30-$250 USD, Jobs: AJAX, Javascript, MySQL, PHP, Website Design)
