Sent EMAIL custom made application (send email as _ _ _ @ _ by sucessslo

I need an application that will allow me to send emails to my customers, from custom email address. What I need is “From” window where do I enter email from which message could be sent “To” where… (Budget: $30-$250 USD, Jobs: AJAX, HTML, Javascript, MySQL, PHP)
