I need a contact form to be added to an existing wordpress site, with EXACTLY the same features as this page:

– Page has some introductory text at the top
1. CONTACT FORM:
– Firstname, Lastname, Email
– Email to be sent to several addresses
– Default text is given, with option for user to add more in the text box
– (If possible) Firstname and Lastname is added to the bottom of the default text when the email is sent.
2. SIDEBAR
– Email sent and target bar-chart is shown
– Facebook
– Twitter
3. SUBSCRIPTION:
Option to Subscribe (Send me updates)
– Developer can use a plugin or own code or variation of the above url.
– Developer must first provide the test page on his own wordpress site. When it has been tested, the developer must set it up on the live site.
– (Optional) contact page is set up to minimise spam
This job must be completed within 7 days of awarding. First draft must be available for testing within 3 days of awarding.
