I need someone to write a form for my website that will allow visitors to send me a credit application. This needs to be similar to forms shown on some of the examples I have included in the links below. I want to be able to take the code you write and simply paste it into my website.

I need someone that is able to modify my website so that each time I list a vehicle, there is a button beside that vehicle that will forward or open up the credit application listed in the job above.
