THis should be a really easy project for a qualified web developer.
I need a three page website developed to collect contact information from people who are getting a free report.
I will provide all of the copy all you need to do is create the theme, pages and load the copy and code from the survey system onto my webserver.
Page one: will contain copy and collect the persons name, email, mailing address and phone number.
Page two: Thank you page will contain copy and a survey.
Page Three: Final thank you page for people who fill out the survey.
The theme should contain a picture of a spine since the site is geared toward chiropractors.
