I need one page created. It is a page to collect email addresses. It should look exactly like these mockups:

Page should be centered, white background bar should stretch 100%
There should be no refreshing between these two screens, the mail form should fade out and the second confirmation should fade in.
It should save all email addresses in a txt file on the server.
