What I need is a small form created that will capture customers contact info and automaticly put it into a Microsoft Excel spreadsheet. In this form I want it to be for a drawing for a $25 gift certificate. It will ask for their full name, full mailing address, telephone and email address. It also will ask for permission to contact them for specials and promotions. I don’t want the bid to be no more than $5 because this is a small project.
