We need this to get proof that client got download
The attached rough daft of the form needs a terms and conditions text box with a compulsory accept or decline option as seen on thousands of other web sites.
I will add the terms and change them from time to time.
Form can be PhP or HTML
I just need the standard agreement “if you accept or decline” box and if they decline their submission of the form is rejected.
