Healthcare Volunteer Project Update

HealthCare Volunteer needs to update their web site with 10 new features to it’s existing web site. High level information and implementation has been attached. Lower Level Details will be attached after acceptance of your bid, so please add 10% extra for any unforeseen expenses in order that you will be able to deliver this project on time. Terms will be as follows:

SERVICES TO INCLUDE: COMPLETION OF ENTIRE ONLINE HEALTHCARE VOLUNTEER PORTAL INCLUDING DESIGN, DEVELOPMENT, IMPLEMENTATION AND QUALITY ASSURANCE TESTING OF WEB SITE. COMPANY HAS PROVIDED A ROUGH DRAFT OF THE FEATURES HEREIN ATTACHED AS EXHIBIT B REQUIRED TO COMPLETE THE ENTIRE PROJECT. THESE FEATURES CAN BE CHANGED AS NEEDED BY THE COMPANY AS LONG AS THE DESCRIPTION OF THE PROJECT DOES NOT CHANGE, DESCRIBED HEREIN: A COMPREHENSIVE GLOBAL HEALTH NETWORK SERVICE THAT ALLOWS DONORS, PATIENTS, VOLUNTEERS, ORGANIZATIONS AND CASE WORKERS TO INTERACT TO FIND AND PROVIDE SERVICES, SUPPLIES, EQUIPMENT AND FINANCIAL RESOURCES TO EMPOWER MEDICAL, DENTAL AND SOCIAL SERVICES AND ASSOCIATED FEATURES.

AS REQUESTED BY CONSULTANT, COMPANY WILL HELP MANAGE PROJECT AND GIVE TIMELY FEEDBACK AS PROGRESS IS MADE AND QUESTIONS ARISE.

CONSULTANT WILL SEND A TWICE PER WEEK REPORT ON NEW FEATURES ADDED SO THAT COMPANY CAN HELP GIVE DIRECTION AND FEEDBACK FREQUENTLY.
IN ORDER TO ENSURE THAT CONSULTANT IS RECEIVING FEEDBACK, INPUT OR DIRECTION CRITICAL TO COMPLETE THE PROJECT ON TIME OR DELIVER REACH A MILESTONE ON TIME, CONSULTANT WILL MAKE ANY REQUESTS FOR PROJECT DIRECTION FROM COMPANY AT LEAST 72 HOURS BEFORE THE INPUT WOULD BE REQUIRED IN ORDER TO STAY ON SCHEDULE AND TO NOT HAVE COMPANY DEEMED RESPONSIBLE FOR PROJECT DELAY. CONSULTANT WILL USE VERSION CONTROL SOFTWARE SO OTHER PROGRAMMERS ON THE PROJECT CAN WORK INTERACTIVELY WITH CONSULTANT.
COMPANY WILL DELIVER AN UPDATED PROJECT PLAN TO THE ROUGH DRAFT BELOW EVERY SUNDAY NIGHT (COMPANY LOCAL TIME (TEXAS, USA)) TO HELP EXPLAIN EXPANDED FEATURES AND CHANGES REQUESTED OF PREVIOUS MILESTONES REACHED AND CONSULTANT WILL BE REQUIRED TO IMPLEMENT CHANGES TO PREVIOUS MILESTONES AND CURRENT MILESTONE IN ORDER TO REACH CURRENT MILESTONE ON TIME.

Payment Terms to be as stated:
Company will pay consultant $____ BID by the completion of entire project.
Company will pay ($10 * Number of days late) per day the project is late beyond February 15, 2011 which will be subtracted out of final two payments to be made. For instance if Consultant is 1 day late subtraction of $10 to occur. 2 days late $10 + $20 = $30 due; 5 days late: $10 + $20+ $30+ $40 + $50 = $150. Payment will be due weekly by company and non-payment after one week will accrue interest at 6% per day.

Project Start:
Start date will be November 15, 2010 and delivery is ON PROPOSED FINISH TIME. Company agrees to provide a minimum of six months of on-going support equal to up to 8 programmer hours per week to support minor changes and bug fixes to the site after delivery date or until features outlined in specification herein are completely functional without errors. Support can be renewed upon mutual agreement between Company and consultant after initial 6 months of on-going support.

Penalties:
HealthCare Volunteer will pay 10% upon completion of each milestone (10 total) and remaining balance upon delivery by FINISH TIME or whenever project is completed minus any penalty payments. Consultant and Company will split any necessary transaction fees or bank fees necessary to make transfer payment to Company, which would be taken out of each payment. Check and Transfer to US banks will not incur any fees and consultant will make its best effort to receive payments via these methods. Lastly final 2 payments will not be made until the entire project is completed to specifications and without bugs. If project is not completed by March 4, 2011, Consultant agrees to refund all monies paid from Company to Consultant to date for this project and any developed product will remain property of Company.

Company has identified 10 milestones to be reached. Company understands that several features are intertwined and that each milestone may only partially be completed by milestone delivery date. No penalty for not reaching the milestone on time (just a delay in payment until the mile stone is reached), and a penalty is incurred only if project is not fully completed and tested by FINISH TIME

10 MILESTONES:
DETAILED REQUIREMENT OF MILESTONES WITH FLOWS WILL BE REQUIRED AND DEVELOPED BY COMPANY AND CONSULTANT. MINOR CHANGES TO THE SYSTEM WILL BE REQUIRED AND EXPECTED, SO LONG AS IT DOES NOT ADVERSELY CHANGE THE SCOPE OF DEVELOPMENT AND FUNCTION, THOSE CHANGES WILL BE MADE ITERATIVELY IN A JOINT MANNER BETWEEN CONSULTANT AND COMPANY.

1. A. UPDATE ENTIRE LOOK AND FEEL AND IMPLEMENT AJAX (DROP DOWNS) AND OTHER USER FUNCTIONALITY FEATURES FOUND ON FACEBOOK AT PRESENT SUCH AS SUGGESTIONS WHILE TYPING IN BOXES ETC. IMPLEMENT CSS 3.0, HTML 5, REDO DROP DOWN MENUS, ENHANCE SEARCH FUNCTIONS, FIX ITEMS ON SITE CURRENTLY BROKEN OR NOT WORKING AND FLOW OF WEB SITE TO BE FIXED.

B. UPDATE ALL COPY AND CONTENT ON THE WEB SITE AS DIRECTED BY COMPANY AND FIX CURRENT GIS SYSTEM FOR VOLUNTEER PROVIDER SEARCH.

2. COMPLETION OF ADDITION OF DONORS (INDIVIDUALS & COMPANIES) FEATURE.

3. DONATION MANAGEMENT/INVENTORY SYSTEM IMPLEMENTED IN ENTIRETY.

4. MESSAGING AND CONNECTION SERVICE BETWEEN (5) TYPES /VARIOUS USER TYPES IMPLEMENTED.

5. COMPLETION OF DONORS TO BE ABLE TO PROVIDE WANTED ITEMS AND ORGANIZATIONS AND VOLUNTEERS TO BE ABLE TO PROVIDE REQUESTED ITEMS AND COMPLETION OF CREATE YOUR OWN VOLUNTEERING APPLICATION FOR ORGANIZATION USERS (MASTER APPLICATION) AND IMPLEMENTATION OF ALL POSSIBLE USES OF THIS FEATURE WITH DIFFERENT USER TYPES.

6. VOLUNTEER TRIPS/ACTIVITIES FEATURES IMPLEMENTED AND CALENDAR SEARCH, ETC. REDO OF SEARCH PAGES AND LAYOUT OF PROFILE PAGES FOR PATIENTS; IMPLEMENT CONFIDENTIALITY / PRIVACY FEATURES FOR PATIENT PROFILES, IMPLEMENTATION OF MASTER VOLUNTEER APPLICATION (COMMON APPLICATION), PLEDGING OF X NUMBER OF TREATMENTS FUNCTIONALITY.

7. ENTIRE FUNDRAISING SYSTEM/FINANCIAL TRANSACTIONS IMPLEMENTED – VOLUNTEERS RAISING FUNDS ON THEIR PROFILE PAGE, REQUESTING FUNDS AS AN ORGANIZATION FROM DONORS.

8. CREATE IN-HOUSE CUSTOMER SERVICE TOOL TO MANAGE AND UPDATE THE ENTIRE WEB SITE SUCH AS LISTINGS APPROVAL, DELETE LISTINGS, APPROVE LISTINGS, APPROVE REVIEWS OF ORGANIZATIONS, EMAIL BLASTS, ETC.

9. IMPLEMENTATION OF PUBLIC HEALTH AID NETWORK – FUNCTIONALITY: VOLUNTEERS CAN SIGN UP TO SERVE IN A SPECIFIC METROPOLITAN CITY. A MANAGEMENT DISASTER COORDINATOR CAN PINPOINT ON A GOOGLE MAP A NATURAL DISASTER AND THE VOLUNTEERS SIGNED UP TO SERVE IN THE CITY ARE DISTRIBUTED AMONG SPECIALTY

10. COMPLETION OF QUALITY ASSURANCE, COMPLETION OF BUG FIXES SENT DURING PROJECT PERIOD AND READY FOR FULL LAUNCH WITHOUT ANY ERRORS TO WEB SITE.
