I need to have a database of office managers of medical practices in the US including those in the home healthcare care industry. Specifically, I need their name, the name of the practice, address, their phone number, fax number, and email address. Other helpful fields would include the type of speciality and the number of providers in the medical practice. So far I have only been able to find sources that provide a subset of these fields (e.g. the attached file lacks the name of the practice and the email address). not sure if I will get bids here or not, but thought it worth the chance to see what people might be able to provide.
