1) Create a system able to create dynamic survey forms and survey steps for each form . Survey information to be captured is medical data.
2) Medical data to be updated by multiple users/user groups
3) Medical data will be approved by super user and can be reedited after approval.
4) Additional medical data will be created using standard formulas shared based on data captured in survey.
5) Reports required to show statistics, patterns of survey values and select preselected reports to issue to patients based on values captured on their system data.
