I need a simple landing page for an affiliate campaign.

In the box that says “questions related to the product they want” I need radio buttons so visitors may select a cpa offer to complete (similar to http://tinyurl.com/29yuwbs)
I have all the content, an image and a testimonial can be found at the cpa website. The content and other details will be provided in a file after bid selected
Design needs to be clean and professional looking.
