We are looking to redesign a PDF that looks very professional, like typical government forms (see N400 pdf as level of quality)

2) the new form must be redesigned in such way that it can be used
a) Print and fill out by pen
b) fill out PDF directly and save and print
c) fill out PDF directly and click EMAIL FORM
d) fill out PDF directly and click print button (not just File->Print)

3) the information doesnt have to be saved to a database (provide an extra quote for that and we might consider doing a database version also)

4) the current PDF was designed for print purposes only and therefore you need to adjust anything that says for example: Circle any of the following to be check boxes instead.

5) you must use proper validation for check boxes, radio buttons etc.
none of the fields should be a required field.
6) Total Pages to be redesigned: 3

7) Page 1 should have a header called: Patient Information

8) page 2 and 3 should have a header called: Medical History

I am promoting my website on Google Ad Words, and I have a site with a php form that sends name, email adress and phone number to my email adress. I also want to be able to the capture a subid (unique number or cookie) of each visitor and then send that information to me with that form, so I can track leads and which keywords the person used to find me .

I have a local plumbing ad, someone clicks on my Google ad, goes to my site read information about me, then goes to the PHP contact form and sends his information to me. I need a system where members can purchase credits and use them to buy time to view a webcam. Once the points are all used up the webcam automatically stops playing. I will need the webcam setup as well. The software for the webcam is already installed. It simply needs to be configured. Lastly, I need the payment system to be set up using Zombaio as the gateway. Here are the remaining details:

1 . I need 3 tip buttons: $1, $5, $10

2. No tip buttons for private rooms. Only a button that directs them to a private page so they can schedule one.

3. If they try to tip and dont have enough credits, they need something that tells them they need to buy more credits.

4. In the web cam software, the user needs to be able to chat with the person on the webcam, but not hear them.

5. If the webcam is not online and they try to access the page, it needs to redirect them to the schedule page.

6. Only members can access this page. No guests.

7. They need the ability to increase the size of the chat 2 x, but if they increase the size, they still need a text box. The chat room:
size of video feed box: 3″ X 3″
size of text chat box: 4″ X 4″

Basically, I need a webcam set up for the site administrator, a “pay-per-credit” system, and I need the credits to automatically withdrawal in relation to how long they watch the webcam. When they run out of credits, the cam will shut them off until they pay for more credits. I also need tips buttons in case the customer wants to tip. The payment processor will be Zombaio and, therefore, the webcam software will need to work directly with this processor.

I have quite a few different graphics I need for a few different websites I am working on. I will seperate them depending upon the website.

1. I am changing the following site over to a different domain and because of that I need to change the logo. Go to http://www.ehypnotherapy.com. I am changing this website over to ChristianHypnosis.org so I need to change the “eHypnotherapy” logo at the top right to “ChristianHypnosis” but I want it to look exactly the same and have the same font. I will need that logo in two or three different file sizes with a transparent background so I can plug it into anything else I need. On the same website, I also would like 3d graphic “software” type box pictures. I do not feel like taking physical pictures of all of my programs again so I want to get 3d box graphics made of all them. This includes the group pictures as well of 4, 5, or 6 cds. I would be fine with you making a single graphic, and showing me how to edit the text, that way you don’t have to give me 50 different single files. I have photoshop.

2. For another website I simply need a logo made and 3d graphics to match. That website is located at meditativeprayer.org. I want a logo based on the “ABC” graphic on TV. Will show you an exmaple. And then I want the 3d graphics to match of course. I belive it is a total of around 9 3d graphics, but I don’t mind you submitted one that I can edit the program name.

3. I am working on another website that is not up yet. I am wanting a logo and 3d graphics that match for it as well. I would like a single graphic that I can edit the product name, and then some group graphics as well of 3, 4, 5 total 3d boxes in the same one. I would also like some website cosmetics done. I guess you could say I want most of the graphics for the website/layout. Not a whole lot, just a header with the logo put into it, and just whatever it takes to make it look really professional. I also will need the logo file in different sizes so I can insert it into different places, etc.

I believe that is all. If everything goes well this could easily be a long term relationship because I do a lot of work online. Please bid on how much you would do for all three of these projects. And ask any questions you would like to.

I need a Very Visually appealing website with a look and feel similar to http://www.myurl.si/PPBOS. You MUST NOT COPY the outlay, but I need the look to be appealing just like this.

What I need.

1. A registration/login function that allows idea seekers to post a profile about what they are looking for. The profile should be searchable on the website.
2. A search function that allow visitors to search the ideas submitted by idea seekers. They must be logged in to perform a search.
3. An internal communication interface (Internal Email) where idea seekers and visitors can communicate with each other.
4. A Logo (Innovative Look and Feel) will tell you what it must say upon winning the auction.
5. A payment function must be integrated. before the idea seeker can post a searchable profile. It could be as simple as paypal for now.
6. In the posted profile, the idea seeker should be able to upload 1 document, but in order for any other user to see the document the idea seeker must “unlock” it for the user requesting to see it.
7. Should be built using an easily updatable CMS.
8. On the backend, I should be able update the website, any posts, remove ideas, etc.

There may and will be changes along the way, so this is an ongoing project. You can build based on these specs.

The solution for this project must be “Ready to Run” Plugin (back/front end) to create a heavy traffic/transactional e-commerce web site with wordpress and the following characteristics:

• All the required functionality to clone groupon.com
• 100% Open code/No obfuscation and secure
• Web 2.0 feel/usability

We are looking for a plugin that implements the functions of groupon.com with some minor changes in mechanics and how it operates (this will be disclosed once a provider is selected). Please familiarize yourself with how http://www.groupon.com works; you can also take a look at, http://www.buywithme.com.

The Plugin should allow us to sell coupons for businesses. The coupon will give the buyer a product or service at a business for a discounted price (the ‘Deal’). However, in order for this ‘Deal’ to occur, a minimum number of coupons must be sold. For example, there may be a ‘Deal’ featured on the site that requires a minimum of 100 people to purchase it before the deal goes ‘live’. If the minimum number is not met, nobody will receive the ‘Deal’ and nobody will be charged for it.

There will be a new ‘Deal’ each day/week, so the member will have to purchase the coupon before the deal expires. At any given point in time, we will only be offering one ‘Deal’ for each category.

In short, we need a back-end system that will give us the ability to provide ‘Daily/Weekly Deals’ to members.

Specifications:

For completeness, I will list some features that will be required for the platform. Of course, these are not the complete requirements and adjustments may be required, so please visit groupon.com to gain a fuller perspective on what is required.

General Plugin requirements :

– all functions from groupon.com and the specified additions
– modular payment-gateway interface to add more payment-gateways
– plugin must be full localized with the wordpress-translation functions (__ and _e)

Administration Rights:

• The ability for an administrator to create a new ‘Deal’ for the category for which they have permission.
• The ability to specify an start date / time, ending date / time, minimum purchase requirements, maximum purchase requirements, and the key terms and conditions (cost per coupon etc.)
• The ability to view all members, their contact details, and coupons purchased.
• The ability to create future cities for my site.
‘Deal’ Features:

• The ability to create separate ‘Deals’ or coupons for each category.
• The ability to set a maximum of the number of coupons that can be sold (a) in aggregate; and (b) to any one member. If the limit is reached before the deal closes, nobody should be able to purchase any more.
• The ability to set a minimum number of coupons that need to be sold before the ‘Deal’ becomes available. If members purchase a ‘Deal’ that does not reach the minimum, they are automatically sent an email stating that the ‘Deal’ closed.
• The ability to restrict the time period for which ‘Deal’ is available (typically 24 hrs to 7 days).
• One the side, the ability to post a ‘Bonus Deal’ or ‘Side Deal’. This functionality would be hidden in the beginning.
• Each coupon will need the following fields: ID Number; Price Paid (by the member); Coupon Value; Short Description; Logo; Address of the Business; Key Terms & Conditions (expiry date etc.)
• For each ‘Deal’, a discussion page in which members can ask questions about the ‘Deal’ and an administrator can login and answer questions for all members to see.

Expired ‘Deals’:

• The ability to select certain expired ‘Deals’ to show on a “Prior Deals” page.
• For each expired ‘Deal’, the ability to show the client’s logo, the ‘Deal’ offered, the number of coupons sold, the total dollars saved.
• Emails:
o The ability to create an auto-generated HTML email, which is sent to all members who opted in for either a site wide or specific category email, alerting them of each day’s ‘Deal’.
o When a member signs up for emails to be sent to them, they should fill out their email address and select which category they would like ‘Deals’ for.
o The ability to create an auto-generated HTML email, which is sent to the member post-purchase, with a link to where they can login and print out any coupons purchased.

E-Commerce Functionality:

• The ability to facilitate payment options with direct credit card transactions or via PayPal.
• The ability to store credit card information for members so that coupons can be purchased once they are logged in.

Member Login:

• The ability for members to login to the site to view and print coupons purchased, credit card information, personal details (name, contact details).
• The ability to import contacts from Facebook, Hotmail, Twitter etc. and refer contacts to our site.
• An affiliate system that credits customers accounts upon the referral of other members (once they make an initial purchase).
• The credit should only be used for the purchase of other offers i.e. cannot be withdrawn for cash.
• Client (Business Owner) Login:
• The ability for clients to login to the site and see total coupons purchased for their specific ‘Deal’.
• The ability to check-off coupons that have already been claimed by a member.
• The ability to print a list of coupons outstanding (not yet redeemed).

Data Collection:

• The ability to collect data for each ‘Deal’: the number of members that viewed the ‘Deal’; the number of coupons sold.
• The ability to download data into a CSV file for any ‘Deal’.
• Integration:
o The ability to integrate the ‘Deal’ each day with Facebook, Twitter and email.
o Search Engine Optimization for Google, Yahoo!, and Bing web searches.

Other:

• The ability for members or business owners to suggest new businesses that they would like to see a ‘Deal’ for.
• Basic information pages: About Us, Contact Us, FAQ, Terms & Conditions etc.
• Overall, the website should be flexible in terms of functional amendment. In the future, the provider may be hired to add or remove certain functions. This will likely be an iterative and on-going assignment.

We are a website development company and do various projects using oscommerce. We have an existing website done in a modified oscommerce. We need a change to it.

Overview:
– we have the site in a development area that you will be required to make the change on
– We have a system for sellers already coded into the website. This allows the website operator to allow sellers to list their products on the website and take a commission from the sale of a product
– We have a report that the website administrator views. This report allows the selection of a date range and a seller and displays the items sold and commission only for items that still have a commission to be paid. see attached.
– the problem with this is the admin does not know which seller has a sale, so the admin must select each seller (and if there are many this will take long) to see if the seller has a sale

Work outline:
– The drop down list entitled “Select Seller” needs to have as one of the selections “All Sellers”
– when a date range and All sellers are selected a list of sellers need to be presented only showing those that have sales with commissions still to be paid for the period selected.
– the admin can then click on one of these sellers and then see the information that is now presented when a single seller is selected.
– We know that after we give you access to the code you may need to revise your bid and time to deliver.

Requirements
– programmer should be already familiar with OSC
– once work is completed the programmer must be available during our work hours (6EST to 2EST) to make quick adjustments as we test and there are any problems. Chat communication and possibly telephone (we will call) will be required for this.

This module purpose aims to build an administration module for managing a contact list of external emails that should be syncronized with registered website users.

Working method

Changes To WordPress Theme

Changes To WordPress Theme
Site is www.jokeo.com, theme is Magazit from Wobzy.

Mainly I selected that because of the “slider” on the homepage.

Issue I’m having is that on pages/articles, I’m posting pictures from Flickr and embedded videos from YouTube, which come through fine on the individual article page.

On the category and main pages, however, the theme posts an excerpt, which includes a thumbnail of the image. But because neither the images nor the videos are hosted by me, the theme doesn’t know what to post.

I’m OK potentially hosting the images if there’s no way to do a thumbnail of a non-hosted image, but for video the theme probably just has to be changed to accomodate the original size of the embedded video on the category and homepage.

I’m fine to potentially even chuck the theme, but I do like the slider.
