Script incompletely loading in WordPress [closed]

I have a script from HireTrakStar that displays available positions in my company. When you click on a position, it shows the job description along with a button for form submission. However, when you click the button, only input boxes without labels are displayed.
I used HTML box element inside Elementor(WordPress) to insert the script on the page.

I was expecting the labels to displayed with the input boxes. Script is working fine until it gets to this form.
