Website Design (html/php)
Simple website design for a medical healthcare practice. Design probably in xhtml/php/css etc. The text information pages should be in html so they are easy to update.
Site must look clean and professional.
About 10-15 pages in total. It will have pictures (which will need edited) as well as text. Must have “contact us” form, embedded google maps for location/directions. Menu with links on left hand side, and some at top. Maybe some minor graphic/logo creation work, e.g. picture slideshow (e.g. animated .gif).

Site developer should be familiar with SEO, and able to provide site preview, for feedback and fine tuning. Once site is done and live, some minor after sales support may be needed but not much.
Time for site completion within 1-2 weeks.
